Middle School Sports Program FAQ for Parents/Guardians

  • All Travel athletes must complete the online registration forms AND have a sports physical completed and turned in at the Nurse’s Office in order to be cleared for athletics.

Parents MUST register their child(ren) for participation in any sport as well as obtaining a sports physical (sports physicals are valid for one calendar year). There will be a registration window open for each season (Fall, Winter, Spring). However, it is suggested that ALL prospective athletes for any season receive their sports physical as soon as possible to avoid any conflicts.

    Preparticipation Physical Examination

Students registering for athletics must have a current physical examination on file in the nurse’s office before they are authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the Nurses’ Office no later than 10 school days prior to the first day of practice/tryouts. Physicals are valid for 12 months from the date of the last exam.

Parents may make an appointment with the doctor, FREE of charge, to get a physical completed before tryout dates. Parents should complete the parent section of the forms to bring with them to the appointment. Athletes are not allowed to try out or participate in a sport without properly completed and approved forms. Remember, there is no charge for this service.
